Get the Most from Your Plan

Your First 30 Days

Register or log ln to My Account. There, you can check your eligibility and claim status, view authorization and update demographics.

Complete the Health Risk Assessment (HRA) to provide more information about your health care needs and earn a Healthy Reward.

Schedule your annual wellness visit with your PCP as soon as you join the plan.

If you have a chronic condition, schedule all your follow up appointments with your specialist, as soon as you join the plan.

Discuss with your providers any questions you have about your medications.

Review your Welcome Kit to learn about all the benefits of your plan.

Answer the Call

Once you’re enrolled, CareFirst health specialists may call to offer assistance. They can help you complete your Health Risk Assessment (HRA) and understand your plan's many benefits to ensure you’re getting the most from your plan.

Finally, CareFirst has partnered with Porter Cares, a local company that may call you to set up an appointment to see you in the comfort of your own home. They can offer services such has home health screenings, access to local resources, help scheduling your doctor visits and more.

We want to hear from you! Once a year, you'll receive a CAHPS (Consumer Assessment of Healthcare Providers and Systems) survey. By providing feedback, you'll be helping us make improvements to your care.

Our Medicare Advantage plans offer a routine dental benefit that provides preventive and comprehensive dental care like:

  • Preventive dental coverage (frequency may vary by service)
    • $0 copay for cleanings, oral exams, x-rays, and fluoride treatments
  • Comprehensive dental coverage (frequency and copays may vary):
    • Comprehensive allowance: $1000-$1500 (varies by plan)
    • Dentures
    • Fillings
    • Root canals
    • Extractions
    • And more!

Access to a large national network of vision retail and independent vision providers:

  • $0 copay for a routine eye exam
  • $10 copay for eyeglass lenses
  • Frame/Contact lens benefit: Select frames from Davis Vision's exclusive collection are covered in full.
    • $200 eyewear allowance for frames or $250 towards contact lenses
  • Savings on lens upgrades and protective coatings
  • $0 copay for routine exam and annual fitting
  • Hearing aid copays: $0-$1475
  • Monthly allowance for members with qualifying conditions to purchase groceries locally, pay for utility bills, including internet and phone, or towards rent or mortgage payments (reimbursement options available).
  • Your Medicare Advantage plan offers a SilverSneakers® fitness benefit which includes gym access and online resources for a $0 copay
  • $0 for 24 one-way rides to and from health-related locations like doctor appointments and to the pharmacy
  • $40 quarterly benefit towards over-the-counter items like vitamins, first aid kits, incontinent supplies and more.
  • $0 exam copay for an annual wellness exam with your primary care physician
  • $0 copay for personalized care coordination and assistance by offering care that comes to you. Your in-home assessment does not replace your yearly visit with your PCP.
