Health Coaching for Disease Management

One-on-one support

If you’re living with a chronic condition like diabetes or chronic obstructive pulmonary disease (COPD), disease management coaching can help you achieve your best possible health. Coaching is confidential and included as part of your medical benefits at no extra cost.

How coaching works

Disease management coaching is personal, phone-based support provided by registered nurses and other specially trained health professionals. Working around your schedule, your coach will motivate and encourage you by providing support for any of the following conditions:

Asthma Reviewing changes in your symptoms, triggers, medications and your action plan.
Atrial fibrillation Reviewing health risks, heart rate and blood pressure monitoring, taking medications as directed and keeping doctor appointments.
Chronic obstructive pulmonary disease Emphasizing the importance of stress management, taking medications as prescribed and recognizing symptoms.
Congestive heart failure Helping you track healthy behaviors such as monitoring your blood pressure and/or weight and reducing sodium. Reviewing your medications, symptoms and any changes in your treatment or action plan.
Coronary artery disease Assisting with strategies to control your blood pressure and eat healthfully. Reviewing your medications, treatment plans, symptoms and activity level.
Diabetes Reviewing your medications, exercise, diet, glucose monitoring habits, potential for complications and more.
Fibromyalgia Assisting with management of stress, fatigue, medication therapy, support groups and more.
Irritable bowel syndrome Explaining the impact of stress and diet, reviewing signs and symptoms, your treatment plan and any medications.
Low back pain Providing support to help you make decisions regarding procedures, treatment and pain management strategies.
Osteoarthritis Evaluating your exercise habits, weight and pain management, need for assistive devices or support groups.
