Health Coaching for Disease Management
One-on-one support
If you’re living with a chronic condition like diabetes or chronic obstructive pulmonary disease (COPD), disease management coaching can help you achieve your best possible health. Coaching is confidential and included as part of your medical benefits at no extra cost.
How coaching works

Congestive heart failure | Helping you track healthy behaviors such as monitoring your blood pressure and/or weight and reducing sodium. Reviewing your medications, symptoms and any changes in your treatment or action plan. |
