Categories of Personal Information
We may collect non-public, personal, financial and medical information about you from various sources, including:
- Information you provide on applications or other forms, such as your name, address, social security number, age and gender.
- Information pertaining to your relationship with CareFirst Associates, its affiliates or other information, such as your policy coverage, premiums and claims payment history.
- Information we receive from other sources such as your employer, your provider and/or other third parties.
How your Information is Used
We will not disclose your health information to anyone outside of CareFirst Administrators unless you have authorized or if we are permitted or required by law. We maintain physical, electronic and procedural safeguards according to federal and state standards to protect your information.
In addition, we limit CareFirst Administrators employee, broker, group health plan administrator, consultant, business associate, provider and agent access to your personal health information to only those who need to know information to conduct business or to you the appropriate products or services.
Disclosure of Your Information
To protect your privacy, affiliated and non-affiliated third parties of CareFirst Administrators are subject to strict confidentiality. Affiliated entities are companies that are part of the CareFirst Administrators corporate family and include health maintenance organizations, third party administrators, health insurers, long-term care insurers and insurance agencies. We only share information with a non-affiliated third party when it is a critical business partner to provide you with service. The information we provide can only be used when we have asked them to for our needs, your needs and/or for your group health plan.
If we collect personally identifiable medical and/or financial information, we will not share this information for any purpose other than the underwriting/administration of an insurance policy or claim, or as otherwise specifically disclosed when the information is collected with your consent.
Privacy policy changes
We periodically review our policies and reserve the right to change them. Regardless of any changes to our privacy policy, we are committed to keeping your personal information secure. Even if you are no longer a customer, our privacy policy will still apply to your records.
