2023 Preventive Incentive Program Details

Who: Perdue employees covered by a CareFirst medical plan who receive their Annual Preventive Health Check-up

When: Between January 1, 2023 and December 2, 2023

Complete a preventive health check-up between January 1, 2023, and December 2, 2023, for a chance to win prize drawings, including:

  • Monthly $50 gift card drawings at all locations
  • A year-end $250 gift card drawing at all locations
  • Three (3) year-end $1,200 gift card drawings company-wide

You will not have to show proof of your exam. Entries will automatically be generated from your CareFirst medical claim. You must be an active employee at the time of the drawing to win the award.

Where can I get a preventive health check-up?

You have options – and as a CareFirst member, your preventive health check-up is $0.

  1. Perdue Wellness Center—get your check-up without clocking out or leaving the building.
  2. Your Doctor’s Office—make an appointment that fits your schedule.
  3. CloseKnit—use a simple app to get your annual check-up virtually. Learn more and register at closeknithealth.com.

What can I expect during my check-up?

Your doctor or a healthcare professional will perform a variety of tests and screenings, including:

  • Reviewing your medical history
  • Checking your vital signs, including blood pressure, temperature and heart rate
  • Listening to your heart and lungs
  • Examining your abdomen, skin and nails, and arms and legs

Your doctor may also request a blood sample to check for conditions like diabetes or high cholesterol. The check-up is also a good time to discuss any health problems you are having.

What is a primary care doctor or PCP?

A doctor or other healthcare professional who provides routine care, coordinates your care with other providers and manages your whole health.

Having a doctor who knows your habits, concerns and medical history often makes it easier to get the care you need, when you need it. And your primary care doctor is the best way to get consistent quality care, which can result in better health and lower costs. Adults in the U.S. with a primary doctor have 33% lower healthcare costs.1
