State Law Enforcement Officers Labor Alliance (SLEOLA)

In an effort to offer the highest level of customer satisfaction and product plans, the State of Maryland has added the following benefit enhancements to the product plans offered to State Law Enforcement Officers Labor Alliance (SLEOLA) members.

Effective January 1, 2024

There are no benefit changes to any plans for 2024.

Medical Plan Benefits

  • No referrals needed. You have direct access to any specialist.
  • No primary care physician needed.
  • Vision benefits are included with medical benefits.
  • Unlimited lifetime maximum.
  • Office visits are covered for minimal copay.
  • In-network well child visits, physicals, and annual GYN exams are covered at 100% with no copay.
  • No paperwork for you. Our PPO physicians submit claims directly to us.
  • Out-of-Area Coverage - You will receive one of the most widely recognized and accepted health care identification cards in the United States BlueCard. So now, you can take your health care benefits with you — across the country and around the world.
  • Advice Line - A 24-hour nurse-line, called FirstHelp, is available when all you need is a little advice.
  • Wellness Discount Program — As CareFirst members, you will also receive access to our Wellness Discount Program, which provides you with discounts on alternative therapies such as acupuncture, chiropractic services, yoga and much more.

By selecting the CareFirst EPO, you drive your own health care decisions by enjoying many hassle-free benefits with this plan. For detailed information about your plan, please see the 2021 SLEOLA EPO Evidence of Coverage (PDF).

Members selecting the EPO plan must select their providers from a national network and must stay in-network to receive coverage benefits.

  • No referrals needed. You have direct access to any specialist.

  • No paperwork for you. Our participating physicians (within the PPO network) submit claims directly to us.
  • Vision benefits are included with medical benefits.
  • No out-of-network coverage.
  • Unlimited lifetime maximum.

  • Wellness Discount Program - As CareFirst members, you will also receive access to our Wellness Discount Program, which provides you with discounts on alternative therapies such as acupuncture, chiropractic services, yoga and much more.

As a POS member, you must select a PCP within your service area (Maryland, the District of Columbia or portions of Northern Virginia) to manage your care. For detailed information about your plan, please see the 2021 SLEOLA POS Evidence of Coverage (PDF).

  • No referrals required for in-network specialists.
  • The flexibility to select any doctor you wish.
  • When you need medical care, you can choose to see your PCP or you can go out-of-network to see any doctor you choose.
  • When you see your PCP, you save money by paying fewer out-of-pocket costs.
  • Each family member must select a primary care physician. Each member will receive an individual ID card identifying the PCP he or she selected.
  • Two levels of benefits in one health plan. At the time you need care, you can visit your PCP or go out-of-network and visit any doctor you wish.
  • Office visits are covered for a minimal copay when care is provided by your PCP or a
    referred specialist.
  • In-network well child visits, physicals, and annual GYN exams are covered at 100% with no copay.
  • Vision benefits are included with medical benefits.
  • No paper work for you. If you receive care from an POS provider, they will submit claims directly
    to us.
  • Out-of-Area coverage is subject to out-of-network deductibles and coinsurance.
  • Advice Line — A 24-hour nurse-line, called FirstHelp, is available when all you need is a little advice.
  • Wellness Discount Program — As a BlueChoice member, you will also receive access to our Wellness Discount Program, which provides you with discounts on alternative therapies such as acupuncture, chiropractic services, yoga and much more.

Mental and Behavioral Health Benefits

Mental Health Services will be provided through CareFirst BlueCross BlueShield.

  • In order to maximize benefits, members should seek services through a participating CareFirst provider. Note: there are no out-of-network benefits in the EPO plan.
  • Inpatient treatment is covered at 100% of allowed benefit. Preauthorization is required through CareFirst.
  • Outpatient services are covered at 100% after a $15 copay.

Vision Benefits

In SLEOLA PPO Plans and SLEOLA POS Plans

Benefits are covered in-network 100% of allowed amount; out-of-network: 70% of allowed amount after deductible.

In SLEOLA EPO Plans

Benefits are covered In-network only at 100% of allowed amounts.

Adult Vision Care—Benefits for Members 19 Years and Older:

You have access to an expanded network of vision providers through Davis Vision* in addition to the BCBS PPO network. Present your CareFirst ID card to use your vision benefits with any participating provider in the expanded network.

*CareFirst partners with Davis Vision to offer an extensive national network of optometrists, ophthalmologists and opticians. Davis Vision is an independent company that provides administrative services for vision care to CareFirst BlueCross BlueShield and CareFirst BlueChoice, Inc. members. Davis Vision is solely responsible for the services it provides.

Members can use PPO and Davis Vision providers to receive in-network benefits for routine vision exams and hardware.

Lenses (once per plan year):
  • Single Vision Lenses — $52 allowed
  • Bifocal Lenses — $82 allowed
  • Trifocal Lenses — $101 allowed

Contact Lenses (in lieu of frames):
  • Contact Lenses — $97 allowed
  • Medically Necessary Contact Lenses — $285 allowed

Pediatric Vision Care: Benefits for Members through Age 18:
